Dr. Patel is a board-certified, fellowship-trained Interventional Pain physician who specializes in treating a variety of chronic pain conditions. He has extensive experience in treating degenerative conditions of the spine, multiple arthritic conditions, cancer pain and other chronic pain disorders. He believes in a multi-modal approach to treatment, which includes non-surgical interventional treatments, medical management, physical therapy and rehabilitation.

Dr. Patel was raised in southwest Virginia and spent significant time in Gwinnett County as a child. He attended the University of Virginia, where he obtained a degree in Chemistry with a specialization in Biochemistry. For medical school, he elected to attend Eastern Virginia Medical School, graduating in 2005. He then completed a residency in Anesthesiology at the University of Michigan Health Systems, one of the top anesthesiology programs in the country. Dr. Patel finished his post-graduate training at the prestigious Interventional Pain Management program at Emory University in 2010.

Since completing his training, Dr. Patel had the honor to serve as Assistant Professor of Anesthesiology and Pain Medicine at Emory where he helped train future pain physicians and participated in research. From 2011 to 2013, Dr. Patel practiced in Griffin and Stockbridge, GA . As a physician there he specialized full-time in treating chronic and acute pain syndromes in both inpatient and outpatient settings. He also participated in innovative research and clinical trials to offer his patients the most advanced forms of treatment.

Dr. Patel now wants to bring his experience in treating the most difficult pain syndromes to his own pain center. His special interventional interests include facet neurotomy for spinal arthritis pain, minimally invasive herniated disc decompression, radio-frequency ablation for neuropathic pain, and spinal cord stimulation for failed back surgery syndrome.

Dr. Patel has presented abstracts at the American Society of Regional Anesthesia meetings, as well as the American Academy of Pain Medicine. Dr. Patel also serves as community faculty at the Philadelphia College of Osteopathic Medicine (PCOM) Suwannee campus. He enjoys teaching the future generation of physicians and giving back to Gwinnett County.

Dr. Patel serves as Chief of Pain Management at DeKalb Medical. He is a member of the American Society of Anesthesiologists, Georgia Society of Anesthesiologists, American Society of Interventional Pain Physicians, International Spine Intervention Society and the American Society of Regional Anesthesia. Dr. Patel serves on the board of directors for Georgia Society of Interventional Pain Physicians. He was recently recognized as an “Industry Physician Leader to Know” for 2016 by Becker’s Healthcare ASC Review. Dr. Patel has been selected to Medtronic Emerging Leader program in the field of interventional pain. He is one of 30 Interventional Pain Physicians selected around the United States. He looks forward to helping patients with chronic pain in Lawrenceville, Lilburn, John’s Creek, Cumming, Braselton, Decatur, Lithonia and all surrounding areas of Atlanta, Georgia.

Dr. Sri Mandyam is a double board-certified anesthesiologist and interventional pain management physician. Originally from Texas, Dr. Mandyam’s interest in pain medicine started in high school while volunteering at the veterans hospital. He received his medical degree from Texas Tech University Health Sciences Center, home of the top-ranked pain medicine program in the United States. He completed his residency in anesthesiology at the University of Texas at Houston, part of the prestigious Texas Medical Center. After his residency, Dr. Mandyam completed an ACGME-accredited fellowship at the University of Utah where he learned advanced, minimally invasive techniques in combating pain including neuromodulation, vertebral augmentation and radiofrequency thermocoagulation. Dr. Mandyam’s philosophy to treating pain utilizes a multi-disciplinary method that focuses on restoring functionality and minimizing the need for invasive surgery. This approach has won him numerous awards, including the Top Doctor Award for Pain Medicine in 2015. In addition, Dr. Mandyam has been selected to the Medtronic Emerging Leaders program in the field of interventional pain medicine. He was one of thirty pain physicians selected in the nation for this highly coveted position. Prior to joining Summit Spine & Joint Centers, Dr. Mandyam was the Medical Director at Advanced Pain Care in Austin, Texas and also served as the Medical Director of the Tulsa Ambulatory Procedure Center in Tulsa, Oklahoma. He is looking forward to utilizing his compassionate approach to health care while serving patients in Forsyth, Gwinnett and DeKalb counties. In his spare time, he enjoys travel, guitar, tennis and spending time with his extended family in North Georgia.

Rajiv Gupta is the Chief Financial Officer at Summit Spine & Joint Centers and joined the team in 2022.  Rajiv is a seasoned Healthcare Finance Executive with over 24 years of financial and operational management experience in investor-owned hospital companies, private equity backed healthcare companies, and publicly traded consulting advisory firms.  Rajiv has extensive strategic planning, financial analysis, data analytics, and M&A transaction experience.  Rajiv’s healthcare sector experience includes hospitals (acute & specialty), physician practices, renal care, home health, hospice, payers,  healthcare IT, and healthcare real estate.

Prior to his current role at Summit, Rajiv was the CFO at Surgery Partners’ owned Bakersfield Heart Hospital (BHH) providing leadership and direction for finance, accounting, patient access, HIM, revenue cycle, supply chain, and data analytics functions for the hospital.  Prior to BHH, Rajiv co-founded a startup renal care company, Innovative Renal Care (IRC).  As SVP, Finance & Analytics for IRC, Rajiv was part of a six-member team that successfully raised $200M in equity from a top PE firm and negotiated $500M in debt commitments from a top 3 US bank.  Prior to IRC, Rajiv held senior corporate finance positions with various portfolio companies of MedCare Investment Funds (a private equity group managing $1 billion in assets) in Nashville, TN and also consulted as a strategic advisor to a few MedCare portfolio companies in San Antonio, TX.

Rajiv earned an M.B.A. in Finance and Operations from Vanderbilt University and a B.B.A. from Birla Institute of Technology & Science (BITS), Pilani, India.

Chief Operating Officer  Keval joined Summit in June 2019. Prior to joining the team, Keval worked as a Senior Financial Analyst at an established interventional pain management practice where he was responsible for data management and analytics, as well as analytical support for operational strategy.  Keval received his B.A. in economics from Emory and his MBA in finance and health care management from Robinson College of Business.

Joe DeBartolo is the Chief Development Officer at Summit Spine & Joint Centers and joined the team in October 2023. Prior to joining Summit, Joe worked as a Vice President on the investment team at MSouth Equity Partners (“MSouth”), a middle market private equity firm based in Atlanta, GA. At MSouth, Joe’s primary responsibilities included deal execution and portfolio company management, focusing on the healthcare and business services sectors. Prior to his tenure at MSouth, Joe worked as an investment banking analyst at SunTrust Robinson Humphrey in the technology and services coverage group. Joe received his B.S. in Business Administration from the University of North Carolina at Chapel Hill.

Chief Administrative Officer Curretta Moon joined Summit Spine & Joint Centers in 2014. Curretta leads ASC operations, provider contracting and credentialing, payor contracting, revenue cycle and information technology teams. Curretta earned a Bachelor’s of Science in Psychology from the University of Virginia. Curretta is originally from Alabama and enjoys watching her daughter play basketball, working out and Auburn football.

General Counsel and Chief Compliance Officer Sean Rosario was named General Counsel & Chief Compliance Officer in October of 2018. Sean brings a wealth of diverse experiences to Summit Spine. Sean has had extensive experience as a private practice attorney and as a U.S. Army veteran. Previously he practiced as a corporate attorney at several large law firms. His professional and academic assignments have taken him to Germany, Bosnia-Herzegovina, Rwanda, and Tanzania, where he served as a Chayes International Public Service Fellow, in the Office of the Prosecutor, United Nations International Criminal Tribunal for Rwanda. Sean is also an adjunct professor at the Emory University School of Law. During his years on active duty, his awards included three Army Commendation Medals, a Joint Service Achievement Medal, two Army Achievement Medals, the Good Conduct Medal, the National Defense Service Medal, the Armed Forces Expeditionary Medal, the Military Outstanding Volunteer Service Medal, the Army Service Ribbon, the Overseas Service Ribbon, the NATO Medal, three Certificates of Achievement, the Expert Rifle Marksmanship Badge, and the Sharpshooter Pistol Marksmanship Badge. He received a Juris Doctor from Harvard Law School, Master of Business Administration from the Indiana University Kelley School of Business, Master of Science in Economics from Purdue University and Bachelor of Science in the Social Sciences from Kansas State University. In addition, Sean has a culinary certification from the Cambridge School of Culinary Arts, a diploma in professional locksmithing and a universal Section 608 EPA Technician Certification in air conditioning and refrigeration systems. He is also a Post Certified Law Enforcement Officer, reserve deputy sheriff with Fulton County and certified firearms instructor. His hobbies outside of work include playing the piano and violin, the culinary arts, and the martial arts, including kali-arnis-escrima, pentjak silat, haganah, and muay thai. He may occasionally bake a mouthwatering Feuilletés de Saint-Jacques et Crevettes au Coulis de Fenouil to sate your epicurean palate.

Dr. Jose Mathew is a board certified physiatrist who is fellowship trained in Interventional Spine and Pain Management. Dr. Mathew received his medical degree from Nova Southeastern University in Fort Lauderdale, Florida where he also obtained a Masters in Public Health. He went on to complete a residency in Physical Medicine and Rehabilitation at Nassau University Medical Center in East Meadow, New York, graduating in 2010. Following his residency, Dr. Mathew completed an Interventional Spine and Sports Medicine fellowship at Alabama Orthopedic Spine and Sports Medicine Associates in Birmingham, Alabama. He has been a contributing author for multiple textbooks and journal articles as well as articles in several local newspapers. He serves as an examiner for the Physical Medicine and Rehabilitation Board exam and is an associate professor at GA-PCOM in Suwanee.

Originally from Georgia, Dr. Mathew has spent most of his life and schooling in the area, attending college at the University of Georgia, receiving his Bachelor of Sciences in both Biology and Microbiology. Dr. Mathew looks forward to serving the community he grew up in and contributing to current advancements in musculoskeletal and spine care, offering them to his patients as they become available.
