Substitute Notice of Data Security Incident

Our practice is posting this substitute notice because we were unable to reach certain patients directly due to insufficient or outdated contact information.
What Happened Our Business Associate, TriZetto Provider Solutions (“TPS”), notified us that it experienced a cybersecurity incident involving one of its web portals used by healthcare providers. According to TPS, it became aware of suspicious activity on October 2, 2025, and an investigation determined that an unauthorized actor accessed certain insurance eligibility verification records beginning in November 2024. TPS informed affected providers, including our practice, beginning on December 9, 2025.
What Information Was Involved Based on information provided to us by TPS, the affected data may have included patient name, address, date of birth, health insurance member number (which may include a Medicare beneficiary identifier), provider name, health insurer name, primary insured or dependent information, and other demographic or health insurance information. TPS has stated that no payment card, bank account, or other financial information was involved, and that it is not aware of any identity theft or fraud related to this incident.
What TPS Has Done TPS reports that it took immediate steps to secure its systems, engaged external cybersecurity experts, notified law enforcement, and implemented additional security protocols to help prevent similar incidents in the future.
What You Can Do Although neither our practice nor TPS has evidence of misuse of information, we encourage patients to remain vigilant by reviewing insurance statements and monitoring for any unusual activity. TPS is offering affected individuals complimentary identity monitoring and fraud assistance services through Kroll.
